A 7 years old HIV infected boy on ART for 2 years presented with altered sensorium. MRI brain showed gliotic changes in the right MCA and posterior cerebral artery (PCA) territory with cortical laminar necrosis with mild atrophy of brain. HIV viral load was 12,100 copies/ml. CSF was normal. At 3 years 4 months of age he had left sided hemiparesis. MRI at that time infarct in right internal capsule with complete occlusion of right main stem middle cerebral artery (MCA). APLA test was positive. Current ART was changed to PI based regimen. However the child succumbed to his illness.
Why was there continued neurological damage inspite of ART?
