GRAND ROUNDS
A 2-month-old female child presented with diarrhea, fever, and perianal swelling for 2 days. She also has a failure to thrive in spite of exclusive breastfeeds. Birth weight was 2.5 kg.. On examination, she was malnourished (weight = 1.8 kg) and dehydrated. There was no lymphadenopathy or organomegaly. The perianal abscess was drained and the child was put on IV antibiotics. Mother’s HIV ELISA test was negative. Investigation showed:
• Hemoglobin = 6.5 gm/dl
• WBC = 5,500 (36% polymorphs. 64% lymphocytes)
• Platelet count = 1,24,000/cumm
• Hypokalemia
• S.creatinine, SGOT, SGPT = Normal
• Serum albumin = 1.8 gm/dl which decreased to 1.1 gm/dl after 15 days
• Total proteins = 3.9 gm/dl
• Blood gases = No acidosis
• CSF = Normal
• Prothrombin time and Partial thromboplastin time = Deranged
• Urine routine = Normal
• Urine electrolytes = Normal
• Stool = Normal
The child was treated with IV antibiotics for sepsis but succumbed after 15 days of illness.
