A 2.5 years old girl presented with diarrhea for 4 months. She had loss of weight, recurrent vomiting, oral thrush along with it. On examination, she was malnourished, had dry scaly skin and pedal edema. Systemic examination was normal. Stool examination showed 30-40 pus cells/hpf and stool culture grew Enterobacter sensitive to polymyxin and colistin. There were also presence of oocytes of cryptosporidium. Her HIV Elisa was positive and subsequently mother was also detected to be HIV infected. She was treated with nitazoxanide and polymyxin and then considered for anti-retroviral therapy.
