GRAND ROUNDS

A 7 year old girl is referred with bilateral parotid enlargement since 2 years. She has had no other problems in past and is going to school with good scholastic performance. Both parents belong to a good socioeconomic status and are well. Father had herpes zoster 4 years back at age of 31 years. She has an elder sister who is 11 years and is normal. On examination, she has bilateral parotid enlargement. Systemic examination is normal. Her weight is 23 kg and height is 116.5 cm. She was tested for HIV ELISA and was found to be HIV infected. The referring doctor could not believe that the child was HIV infected as she was otherwise asymptomatic and from a good socio economic background and had never received any blood transfusion or hospitalization.
