A 10 years old HIV infected child was on antiretroviral therapy (ART) since the age of 5 years Her ART consisted of Stavudine (d4T), Lamivudine (3TC) and Nevirapine (NVP). She now presented with jaundice. Hepatitis B and C markers were negative. Ultrasound showed fatty liver. She subsequently had vomiting and severe metabolic acidosis (pH = 7.09, bicarbonate = 3.7 mmol/L).
