GRAND ROUNDS
A 10 years old girl had polyarthritis involving all joints except hips and shoulders along with intermittent fever for part 1 year. At onset of symptoms, she had diarrhea with blood and mucus. She was suspected to have inflammatory bowel disease (IBD) though colonoscopy was normal and started on sulfasalazine but had no response in joint symptoms. She was subsequently tested for HIV after 1 year and found to be HIV infected. Her mother was also detected to be HIV infected. Her CD4 was 198 (7%). ANA, RA factor, Hepatitis C and HBsAg were negative. ESR was 119 mm at end of 1 hour.
