GRAND ROUNDS
A 12 years old girl with vertically HIV-1 infection on antiretroviral therapy (ART) for past 8 years presented fever and diarrhea for one week. Initially, she was on Zidovudine (AZT) and Lamivudine (3TC) for 2 years and then was shifted to Stavudine (d4T), Didanosine (ddI) and Efavirenz (EFV) which has been continued till date. Her CD4 count has remained normal over this period and last HIV viral load done 2 months back was 400 copies/ml. On examination, she had tender hepatomegaly. Investigations showed:
• SGOT = 532 IU/L, SGPT = 484 IU/L.
• USG Abdomen = Fatty liver
• VBG = pH = 7.29, Bicarbonate = 15.9 mmol/L
• Serum cholesterol = 172 mg/dl, Triglyceride = 340 mg/dl.
