GRAND ROUNDS
An 8 years old HIV infected boy presented with neuroregression for past 6 months. Initially, he was unable to walk properly, followed by frequent falls and now the child was bedridden. Subsequently, he lost bladder and bowel control and speech has now come down to monosyllables. However, he is able to communicate with the parents. He was on Zdovudine (AZT), lamivudine (3TC) and Nevirapine (NVP) for the past 3 years but these medications had been stopped for the past 6 months. Both parents were also HIV infected. On examination, he was alert, had generalized lymphadenopathy, with failure to thrive (Weight = 16 kg, <5th centile; Height = 110cm, <5th centile) with pallor. On CNS examination, he had clasp knife spasticity, power of 3/5 and brisk deep tendon reflexes with extensor planters. There were no other sensory or focal neurological deficits. Other systems were normal.
