A 2 years 9 months old boy presented with progressive weakness of both upper and lower limbs since 4 months and change in voice since 3 months. He was on antituberculous therapy (ATT) since past 6 months in view of pulmonary TB. On examination, he had hypertonia with head lag, power of 3/5 in upper limbs and 2/5 in lower limbs with brisk deep tendon reflexes. MRI brain showed generalized cortical atrophy and MRI spine was normal. CSF examination was normal. HIV ELISA was reactive and both parents also subsequently tested positive for HIV by ELISA. His CD4 count was 843 (10.9%).
