A 10 years old HIV infected girl had abdominal distension for one month and ultrasonography (USG) of the abdomen showed mild hepatomegaly & splenomegaly with ascites. Liver function tests (LFT) were abnormal and remained abnormal for next 6 months. HBsAg, Hepatitis C ELISA, ANA, Anti liver kidney microsomal (Anti LKM) antibody and anti smooth muscle antibody were negative. Work up for Wilson's disease was negative. Serum alpha 1 antitrypsin was normal. She was started on ART. Three months later, her liver enzymes gradually decreased.
