GRAND ROUNDS
A 10 year old boy was diagnosed to be HIV infected. He had fever, weight loss, anorexia since 3 months and both parents were HIV infected. On examination, he had generalized lymphadenopathy with splenomegaly. Other systems were normal. Weight was 18 kg. His CD4 was 270 cells/cumm (15%) and CD4:CD8 was 0.19. Hemogram showed hemoglobin of 12.4 gm%, WBC count = 10,900/cumm (68% polymorphs, 30% lymphocytes) and ESR of 100 mm at end of 1 hour. He was started on 3 drug antiretroviral therapy (ART) consisting of Zidovudine (AZT), Lamivudine (3TC) and Nevirapine (NVP). He was alright for next 5 years and CD4 count improved to 420 cells/cumm (21%). Subsequently he developed fever, anorexia and weight loss for 2 months. Investigations showed a right lower zone pneumonia with hemoglobin of 6.3 gm%, WBC count = 1000/cumm (Absolute Neutrophil count = 160/cumm) and ESR of 70 mm at end of 1 hour with thrombocytopenia.
What could be the cause of pancytopenia in this child?

Q. i have a 2 months old in my OPD,2.2kg,male born at 32 weeks,otherwise thriving well infant with hiv positive mother who caught it through a blood transfusion during an MTP app. 2 yeras back. the baby is on top milk by spoon on syp. Zidovudine since birth. Till now baby has not investigated for hiv -kindly suggest me- 1.what invesigations are to be done when 2. vaccination schedule for this baby.
