A 5 years patient defaulted on anti-retroviral therapy (ART) after 2 years of treatment with zidovudine (AZT), lamivudine (3TC) and nevirapine (NVP). At 6 years of age, he had pleural effusion with perihilar haziness. Sputum smear showed presence of acid fast bacilli. He was started on ATT consisting of isoniazid, rifampicin, pyrazinamide and ethambutol. Patient cannot afford Rifabutin.
